What do I need to bring for proof of
eligibility?
You must give certain information before your MEDI-CAL can be approved.
You may apply without proof, but you would have to provide it within a
few days. The following is a list of items you will need to provide (some
take a while to retrieve):
* Social Security card(s)
* Medicare card(s)
* Naturalization document(s)
* Alien registration card(s)
* Pregnancy verification
* Income verification:
Employee pay stubs or a statement from your employer showing gross
earning and deductions
Award letter or checks showing amount of pension or benefits, including
Social Security and V.A.
State Unemployment or Disability award letter
Student Loan grant award letter(s) or loan grant papers
Statement from providers of other income (contributions, refunds,
child support, etc.)
Self-employment information: Last year's tax return or current
ledgers, current inventory, including business equipment and supplies
* Care costs for child/incapacitated person(s)
* Property Tax statements for all property
* Vehicle Registration(s) for automobiles, boats, campers, and trailers
* All checking and savings account statements and trust account documents
* All stocks (brokerage statements), bonds (including U.S. Savings bonds)
and mutual funds
* All deeds of trust, mortgages, other promissory notes and contracts
of sale
* All life insurance policies, including cash surrender value
* All annuity policies
* All burial trusts/prepaid burial contracts/information on burial plots
* Documentation regarding current value of all trusts
* Payment books(s) for all encumbered property
* All policies/cards for health insurance you currently have or which
are available to you
* Application(s) for possible available income (i.e. unemployment benefits,
state disability benefits)
* Court orders relating to income and property
* Lease agreements
* Life estate documents
* Copies of patient trust account ledgers
* Rent receipts, current utility bills, or housing statement
* Copies of child support orders or divorce decree
* Social Security disability or SSI denial or discontinuing notice (if
applying for disability-based MEDI-CAL)
* Evidence of California residency
