Eligibility
1. The child must be under 21 and must have a CCS medically eligible condition as determined by the CCS medical consultants. Medically eligible is defined as "a person who has physical defects resulting from congenital anomalies, or acquired through disease, accident or faulty development.

2. Families must apply in their county of residence. Undocumented children may be eligible for services.

3. The family adjusted gross income must be under $40,000 as shown on their latest California Income Tax return or equivalent. A family with an income over $40,000 may be eligible if the cost of medical care of the CCS medical condition is over 20% of the family's Adjusted Gross Income. There are no financial eligibility restrictions for diagnostic or MTU services.

4. The family must be willing to apply for Medi-Cal if referred by CCS staff. Children with full-scope Medi-Cal (including those in Foster Care) are financially eligible.

5. The family must use their health insurance to the fullest extent of its benefits.

6. The family must pay a $20 assessment fee if their income is over 100% of the federal poverty guidelines, and the family does not have full scope Medi-Cal.

7. Families with an income over $25,000 must pay an enrollment fee based on family income and size. (For instance, the fee for a family of four with an income of $39,000 is $120).

Residential/financial eligibility is reviewed yearly and fees are assessed on an annual basis.
